INFORMED CONSENT AGREEMENT
Measuring mental health literacy and competence: An on-line introduction to eating disorders workshop
You are being asked to participate in a research study. However, before you give your consent to be a volunteer, we want you to read the following and ask as many questions as necessary to be sure that you understand what your participation will involve.

PURPOSE OF THE RESEARCH
The purpose of this study is to develop an on-line introductory course on eating disorders and investigate the levels of mental health literacy and clinical competency toward eating disorders of licensed MFTs and registered MFT interns before and one month after the workshop.
DURATION OF PARTICIPATION IN THE RESEARCH
A total of 240 participants will be involved in this study. The duration of your participation in the study will be up to 90 minutes over a three to five week period.
PROCEDURES TO BE FOLLOWED DURING THE RESEARCH
If you agree to participate in this research, the following will be the step by step process.
You will access the study through the website http://www.Daniellebe.com. The website will show the inclusion criteria on the first page. If you meet the criteria to participate you will be taken to the informed consent page. You will be instructed to read the informed consent page. After reading informed consent and if you agree to participate, click on the “I accept” button. You can download a copy of the informed consent and/or the Subject’s Bill of Rights by clicking on the corresponding links. After agreeing to participate, you are asked demographic questions such as age, gender, ethnicity, education, eating disorder education, and eating disorder experience. Next, you are asked for your e-mail so that you can be contacted in three weeks to complete the follow-up surveys. A follow-up reminder e-mail will be sent after four weeks of completing the workshop. The first questionnaire is Attitudes and Beliefs about a Mental Health Problem: Professional Views (Hay, Darby, & Mond, 2007; Mond, Robertson-Smith, & Veter, 2007) and consists of a vignette with several questions. The next questionnaire is 24 true/false questions about your competence in a specific mental disorder (Rosenvinge, 2003). The workshop is exactly one hour and will require access to the Internet and your full attention. In three weeks you will be e-mail instructions on how to complete the follow-up questionnaires. Again you will complete the Attitudes and Beliefs about a Mental Health Problem: Professional Views and the competence questionnaire on-line. Instructions for receiving one CEU and participating in a drawing for one of three $50 Amazon gift cards will be provided on the announcement webpage after completing the follow-up questionnaires. The researcher’s contact information will be listed in case you have comments and/or questions. The total time for completing this study is 90 minutes over a three to five week period.
RISKS
We believe that the there are no risks for participating in this study. If you do experience emotional distress contact the researcher for referrals. If you experience extreme emotional distress call the crisis line at 1-800-479-3333.
BENEFITS OF THE RESEARCH
You will learn about eating disorders and we hope to learn more about marriage and family therapists’ mental health literacy and clinical competency toward eating disorders. Your participation may help individuals and families with eating disorders in the future. Additionally, you will be able to receive one CEU (if you are registered with the CA licensing board, BBS) for completing the questionnaires, workshop, and follow-up questionnaire. The CEU is free of cost. You will also be able to participate in a drawing for one of three $50 Amazon gift cards after completing the follow-up surveys.
CONFIDENTIALITY
All identifiable information will be kept separate from your scores. Identifiable information will never be published or used for any other use beside sending a follow-up e-mail reminder and providing you with one CEU and information about participating in the drawing. The results of this study may be published in scientific journals, or be presented at conferences as long as you are not identified and cannot reasonably be identified from it.

SUBJECT COST or COMPENSATION FOR PARTICIPATION
Your participation is voluntary. You will be compensated with one CEU if you are registered with the CA licensing board (BBS).
SUBJECT RIGHTS AND RESEARCH WITHDRAWAL
Your participation in this study is voluntary. If you choose not to participate in this study, there will be no penalty or loss of benefits to which you are otherwise entitled and your relationship with Alliant International University will not be affected. In addition, you may discontinue participation at any time without any penalty or loss of benefits. You may also refuse to answer any questions you do not wish to answer.
We have tried to explain all the important details about the study to you. If you have any questions that are not answered here, the investigator will be happy to give you more information.
